Advisor/Department Agreement Form
This official University document is used to determine student organization classification as defined by University Rule 6-401A. Depending on the information provided, an Advisor may be required for their Dean or head of the department sign in the first blank to take oversight and responsibility of their organization OR sign in the second signature space declaring the organization separate independent operation. This second signature space declares the organization is REGISTERED and operates as a separate legal entity with no on-campus affiliation nor sponsorship. The classification is determined by organization relationship to the department if one exists.
