This form must be completed by all Club Sports Participants prior to being involved in any club activity. The form needs to be filled out electronically each year. Students participating in multiple activities may be asked to fill out multiple waivers acknowledging the risk for each activity. .IF YOU ARE BUILDING A FLOAT! The deadline to register your organization in the float building competition is Wednesday, September 14th at 5pm. This year we will have 3 judged classes that students can take part in. Large floats, small floats and themed walking units. The first 10 approved designs in each group will receive funding. This form is to be used by any organization that would like to request athletic facilities. PLEASE NOTE. You are not approved to use the space until you get a conformation email.
This form will allow you to request multiple days in the same facility. If you wish to request multiple facilities you will need to do a different form for each facility.
