Student Organization Event Guest List
If your organization is hosting an event at which alcohol is a component, this guest list form (including guests names, birth dates, and ID numbers) must be submitted to SAGA no less than 3 business days prior to the event.
At any organization event involving alcohol, the hosting/sponsoring organization must designate a willing and sober representative that will remain present and abstain from consuming alcohol for the duration of the event.

- Sober Representatives contact information
- At any organization event involving alcohol, the sponsoring organization must designate at least 2 sober representatives. After 2 representatives, there should be 1 representative for every 20 additional people. A sober monitor is a willing and sober individual that will remain present and sober for the duration of the event. The sober representative’s name and contact information must be submitted to SAGA at least 3 business days before the event.

- Guest list document upload. Must include each guest's name, LUC Student ID number. and birth date (including year)
