2017 Legacy Awards Nomination Form

DEADLINE EXTENDED! Nominations must be submitted by 5:00 p.m. on Friday, March 10, 2017.

It is recommended those with a CI Sync account log in to their account to complete their nomination(s).  If you are logged in while completing your nomination you will have the option to save your nomination at any point, and review it before submitting.  If you are accessing this form as a public user you are not able to save your entry.  All CI students, faculty, staff, and administrators may set up a CI Sync account through their MyCI log-in, with CI Sync being an available application on the left side menu.  This nomination can be found under 'Forms'.

Purpose 

Hosted through a collaboration between Associated Students Inc. and Student Leadership Programs, The Legacy Awards: A Celebration of Leadership is CI's premiere recognition of students, student organizations, faculty, staff, and administrators who live the University mission through a commitment to student-centeredness, service, community and individual development. 

Recognition Categories

  • Dr. Richard R. Rush Outstanding Senior Student Leader Award

  • Outstanding Student Group Award

Eligibility

All student nominees must have a 2.5 prior semester and cumulative grade point average and be in good student conduct behavioral standing.

Nomination Guidelines & Instructions

  • Please submit a separate nomination form for each individual, student organization or group.
  • You may nominate an individual for multiple awards using the same form, but only complete entries for each specific award will be considered.
    • Please note that a nomination for one award does not automatically nominate the individual for the other award. You must completely fill out entries for each specific award you have selected.
  • Faculty, administrators, staff and CI students may nominate CI students for the student Maximus Award and/or Student Leadership Awards.
  • Only CI students may submit nominations for the staff, faculty, and administrator Maximus Awards.
  • Faculty, staff, and administrators may not nominate other faculty, staff or administrators for any award - they may only nominate students. 
  • All responses should be submitted in paragraph format with a recommended minimum 100 words per response.
  • Partial/incomplete nominations will not be considered.  
  • Self-nominations will not be accepted for individual awards. 
  • Fraudulent nominations will not be accepted.
  • Only nominations received by the deadline will be considered.
  • Multiple nominations submitted for one individual, student organization or group does not guarantee that individual or group as the winner of that category.  Nominations are considered based on quality, depth, and breadth, not quantity of submissions.

Nomination Review Information

  • The selection committees will score the degree to which a nominee meets the award criteria based on the quality of your nomination, therefore, it is recommended that you use specific examples and detailed responses as it will greatly increase the consideration given to your nomination.  Responses to nomination questions should be submitted in paragraph format and it is recommended that each response be a minimum of 100 words to ensure depth of response.
  • In reviewing nominations, the Student Leadership Awards are conducted through a 'blind review' in that the nominee's name is removed from selection materials before they are given to the selection committee members.  This allows the selection committee to consider the weight of the nomination with as little influence of personal knowledge of the nominee as possible.  In order to facilitate this process, for the Student Leadership Awards please do not use the student's name in your responses, however use of 'he/she/they' is appropriate.  The Maximus Awards do not facilitate a blind review, and so inclusion of nominee name in responses for that section is allowed.  This information will be repeated on related sections of the nomination.
  • Students nominated for an Outstanding Student Leader Award must accept their nomination and submit a resume and response to three short-answer questions in order to be considered for the award.  This process allows students to reflect on their leadership identity and the impact of their leadership experiences on themselves and their community.  Nominees will be contacted by Student Leadership Programs with instructions on how to submit the required information .
  • Please note that should your nominee be selected as an award recipient, the selection committee may share your name and/or comments with your nominee.  Your comments may also be used to honor your nominee during the actual event as the nominees are announced.
