SLL Event Planning Request
Note: If you are requesting for an event that is not during regular business hours, please complete an After Hours request and submit this request at least two weeks prior to date of event.
Before you start this form you will need your advisor's email address, please make sure you use the correct one or your request may not be completed. If you do not use your advisor's email, your request will be denied.
This is the form to use for reserving space for a meeting, activity or event, and If any audio/visual or other equipment is needed. Please check back if you have not received a confirmation or denial within 7days.
Please give us at least 72 hours (3 business days) notice on any request (except on After Hours events).
