Alcohol Registration Form
1. Alcohol will be present
2. A contract(s) must be signed with an outside company or group that will be providing/serving alcohol
Applications for alcohol use must be submitted at least ten (10) days in advance of the event. Upon approval of the event, a representative of the organization MUST meet with the CSC Special Events Coordinator or a representative of the Dean of Students at least five (5) days prior to the event to complete an alcohol use checklist .
Note - Registration does not constitute approval of event.
