Scheduling Request Form
This form is intended to be a "One Stop Shop" to be completed by any recognized student club/organization, athletic team, or department looking to schedule a program or activity on or off campus.
This form will also serve as the official registration form for all campus Fundraisers conducted by student clubs and organizations, as well as an official request for SIP Grant Funding through the Student Activities Organization.
Failure to complete this form may hinder the successful implementation of your program or activity.
