2017-2018 Leadership Award Nomination Form

Submit a typed letter of support for an individual or group whom you feel is deserving of one of the awards listed. The typed letter (one page or less, single-spaced) should explain why your nominee(s) is/are worthy of the team’s consideration. A Co-Curricular Portfolio or supporting documentation can be attached, but actual nomination may be no more than one typed page in length.

You may self-nominate. Nomination letters are due NO LATER THAN Wednesday, March 28, 2018 no later than 11:45 p.m.
