To assure you and your Club/Organization have a smooth and successful trip we have compiled a list of things you will need to do.

PLEASE NOTE: All of the following documents must be turned in for campus approval. Please make sure your organization completes all of the required documents 1 month prior to departure. 

  1. Activity Petition approval email from OSA
  2. Approved Travel Application
  3. Copy of your insurance card
  4. University Liability Waiver/per participant
  5. Medical History/Treatment Form/per participant
