2017-2018 Recreation Club Coach/Instructor Application

Coach/Instructor Eligibility

Each 2017-2018 Recreation Club Coach/Instructor Application will be reviewed and approved, deferred, or denied, by the Sport Clubs Advisor or representative. An individual is not cleared to coach/instruct until their application has been approved, they have submitted the following forms to Student Organizations & Leadership (University Union - 2nd Floor), cleared the online background check, and received final approval from the Sport Clubs Advisor or their representative.

  1. HR Volunteer Identification Form
  2. Coach Contract & Agreement Form (must be signed by the club president & treasurer)
  3. Concussion Management Certificate (for high risk recreation clubs)
  4. CPR/First Aid/AED Certificate (for high risk recreation clubs)
  5. Discipline specific certifications (if applicable)
