Community Action Completion Form

The Community Action Completion Form for the 2013-2014academic year must be submitted by Organizations two weeks after the completion of their Community Action Project. Contact information from the on-site supervisor of the organization or the contact person of the organization the service was completed with must be provided on this form. Organizations who complete their CAP by March 3rd by 5 PM will be reviewed for recognition at the Student Life Leadership Awards. All other Community Action Projects must be completed, with their form submitted, by May 2nd by 5 PM.  Don’t forget to include pictures of the project!
