Academic Misconduct Action
Section 10.1 of the Student Code of Conduct addresses academic misconduct.
Section 10.1A allows the instructor to administer the misconduct and take sanction action up to and including failing the student in the class. As such, the student must be provided information on the academic misconduct process (section 10.1) and notification of the role of the university ombudsperson.
By completing this form, the Dean of Students Office will notify the student, through email to the student's AccessID, of the academic misconduct action. The course instructor and department chairperson will be notified of the communication to the student through an email cc. The website links for Section 10.1 (explaining due process) and the role of the university ombudsperson will be included in the email.
If the instructor wishes to take further action (e.g. transcript notation, record notation, probation, suspension, expulsion), then Section 10.1B must be followed, which involves Student Code of Conduct charges being filed. This is referenced and option provided at the conclusion of this form.
