Student Travel Approval Form
Please use this request to provide details of the travel you or your group would like permission to undertake. This form needs to be submitted no less than 14 days business days prior to travel. Please see the travel file (OrgSync/files/Travel Information) to see needed student group travel information before beginning your request. Before you begin, please have ready to submit or upload -
- A detailed travel itinerary or agenda including travel dates and times of your trip
- A list of who is traveling with S numbers (please indicate which are staff/faculty/students)
- An itemized travel/conference budget, where travel involves a student club or organization.
- In the event of an overnight trip, provide name, address and phone number of where you are lodging.
This form is sent to the Director of Student Life and Leadership for review. If approved, you will receive an email stating your travel is approved and you will need to submit SLCC Travel Conduct Agreement, Waiver and Releases for each member of the group, there are separate ones for students and advisors/faculty. To access these forms, travelers must log in to SLCCSync (directly or through MySLCC), and they must be submitted no less than seven days business prior to travel.
