SLCC Advisors/Faculty/Staff Travel Conduct Agreement, Waiver and Release

NOTE: Only the Advisor/Faculty/Staff member traveling may submit this form, others will be denied.

This agreement is used by an advisor/faculty/staff member participating in SLCC student travel. It outlines the responsibilities and expectations of everyone traveling as a member of the group. By following the pre-approved agenda, this travel conduct agreement, and the policies and procedures of SLCC, you can potentially reduce personal liability, and increase personal and group safety. It is expected the advisor/instructor/staff member will read the travel guidelines prior to this meeting, so you have the correct information to share with your group

Advisors/Instructors- you are required to conduct a meeting prior to travel to review the safety and liability issues for everyone involved in this travel.
