2017 Leadership Awards Nomination Form

This year's annual Leadership Awards Reception will be held on Wednesday, May 3, 2017 in the Great Hall of the Memorial Student Center.  This reception serves as a time to recognize and honor all of the truly wonderful and talented leaders on the UW-Stout campus. 

To make your nomination, please complete the form below.  If you would like to nominate an individual or organization for more than one award, please complete a separate form for each nomination. 

This year's reception will include eight (8) awards, plus the Samuel E. Wood Medallion Award, the highest non-academic award given at UW-Stout.  Please note there is a separate form for the Samuel E. Wood Medallion Award and it is self-nominated.   More information on this award can be found online at

This year's awards are:

1. Volunteer of the Year

This award recognizes a student  who has dedicated time, energy and resources to make a lasting impact on the community through volunteer efforts. This student should exemplify a character of service and giving.

2. Advocate of the Year 

This award is given to a student or organization who has shown passion, courage and resilience to bring about change on an issue.  This student or organization boldly speaks out in support of a cause affecting Stout and the community.

3. Student Staff Team of the Year

This student staff team shows a great deal of professionalism, service and knowledge to the campus.  This group of staff can always be counted on to go above and beyond their normal job description.

4. Student Employee of the Year

This student is one of the best....the employee that everybody wants and strives to be.  This student staff member shows a great deal of potential and leadership abilities that will make them a success in the real world.  Note: Graduate assistants are not eligible for this award.

5. Org Advisor of the Year

This award goes to an organization advisor who knows and understands the importance of advising.   This advisor provides opportunity, encouragement and motivation to the students so that they can lead, learn and succeed.

6.  Student Organization Event of the Year

This award goes to a student organization event that was open to the entire campus between April 2016 and March 2017.  The event can be service related, social or collaborative.  This event has made a positive impact on the overall campus life and culture at UW-Stout.

7. Student Organization of the Year

This award goes to a student organization that has made outstanding contributions to student involvement and positively impacted the campus community.  This student organization continuously lives their mission and has made a name for themselves on campus. 

8.  Outstanding Student Leader of the Year

This student exemplifies true leadership qualities and has made a positive and lasting impact on the Stout community.  This award should go to a student who has chosen to better themselves through leadership development opportunities and who without a doubt, will continue to succeed and lead wherever their life takes them.

All nominations must be made no later than Friday, March 10th at midnight.  After nominations are collected, a selection committee comprised of students, faculty and staff members will meet to review all nominees.  Invites will then be sent to all nominees to attend the Leadership Awards Reception.
