Advisor Agreement Form 2017/2018
This form is for each organization's advisor to complete prior to each organization being recognized through the Student Organization Registration process. The form is automatically sent to the advisor listed in an organization's portal.
Please note that students should not be completing this form for their advisor. It needs to be completed by the full time faculty or staff member who has agreed to be your advisor.
This form will also need to be completed if there is need for your organization to change advisors during the school year.
