Organziation Reinstatement Request

This form is for disabled organizations wishing to reform at Texas Tech. This is not a form for current student organizations who need to re-register.

Once you organization has failed to register for the current academic year or complete the required risk management training, your organization profile will be disabled and no longer recognized by the University.

In order to reinstate your organization and gain University recognition you must fill out the following form and meet with the Student Organizations Coordinator in the Center for Campus life. Once your form is submitted you will be contacted to set up an appointment.

If you are not contacted with in 3-5 days please follow up with the Center for Campus Life at (806)742-5433.
